How Life Insurance Underwriting and Health Classes Work
Last updated September 2026
Underwriting is how an insurer turns your health, lifestyle, and financial information into a price — and understanding it makes the process far less mysterious.
What Underwriters Actually Look At
- Medical history and current health conditions
- Height, weight, blood pressure, and basic lab values
- Tobacco and nicotine use
- Family health history, in some cases
- Lifestyle factors like hazardous hobbies, driving record, or occupation
Rate Classes
Insurers sort applicants into rate classes — often named things like Preferred Plus, Preferred, Standard Plus, and Standard, sometimes with separate tobacco tiers — each with its own pricing. Two people with the same diagnosis can land in different classes based on how well the condition is controlled, and the same applicant can be rated very differently from one carrier to the next, since every company weighs factors a little differently.
Full, Simplified, and Guaranteed Issue
Fully underwritten policies (the traditional path, often including a paramedical exam) generally offer the best rates for healthy applicants. Simplified-issue policies skip the exam and ask a shorter set of health questions, trading some cost for speed. Guaranteed-issue policies ask no health questions at all, in exchange for lower coverage amounts and higher cost per dollar of coverage — a fallback option when health history makes the other two paths harder.
